Soccer Participation Waiver and Release of Liability Agreement

This Agreement is made between, the parent or legal guardian of, a minor (the "Player"), and “Soccer Doctor”.

1. Acknowledgment of Risk
I, the undersigned, hereby acknowledge and understand that participating in soccer and related activities inherently involves risks, including the risk of serious injury, permanent disability, or death. I acknowledge that injuries may occur in any sport, and while precautions will be taken, these risks cannot be fully eliminated.

2. Assumption of Risk
I understand that by signing this Agreement, I am voluntarily allowing the Player to participate in soccer activities organized by Soccer Doctor. I accept and assume all risks of injury that may arise from participation in such activities, including, but not limited to, physical injury due to contact, falls, or other accidents, and acknowledge that I have made the decision to allow the Player to participate with full knowledge of these risks.

3. Waiver and Release of Liability
In consideration of the Player being permitted to participate in the soccer activities, I, on behalf of myself, the Player, and our respective heirs, executors, administrators, and assigns, hereby release, discharge, and hold harmless Soccer Doctor, its officers, employees, coaches, volunteers, agents, and affiliates from any and all claims, demands, actions, causes of action, and liability arising out of or related to any injury, loss, or damage (including but not limited to personal injury, medical expenses, or death) sustained by the Player, whether caused by negligence or otherwise, during or in connection with participation in the soccer activity.
